Expiration dates are tricky.

For a manufacturer it is (usually) :
1) when 10% of the active ingredient has degraded to (something else)
2) or when they have done studies for "long enough" for the shelf-life they want.

It sounds like Epi-pens fall into the second category.

Stability studies are done at increased temperatures and humidity to see how long something is stable. These more rapid degradation studies can be easily extrapolated to stability at room temperature and normal humidity levels.

On the medication label for medications from your pharmacy (if it is in North Carolina) is:
"If the prescription is dispensed in a container other than the manufacturer’s original container, a discard date, which shall be the earlier of one year from the date dispensed or the manufacturer’s expiration date, whichever is earlier."

The pharmacy wants you to check in with your doctor, at least once per year, to make sure the medication is still appropriate.
