Hi Ted,
Is there some reason why you wouldn't want to cut and paste the text from one of our online licenses into the text of your book? Or are you looking for a specific format for the art, perhaps? If this doesn't already exist, it sounds like a great idea. I'll look into it.
Regarding derivative works, yes, you can simply check "no" and your license will not allow your work to be modified.

The license I'll use is the one that requires attribution and prohibits derivatives. I'd like to be able to say somethin in the book to the effect that the license does allow users to copy the text freely so long as it is unaltered, etc. but I wondered whether you were planning to give or approve some language to that effect?
