How does this work? Most works that currently are under CC licenses
are not likely sold by themselves, but re-used through inclusion in
other works: an image inside a book, brochure, flyer/leaflet, music in
a collection or as background to video.

Is a promotional brochure of a non-profit organisation, where the
non-profit pays for the distribution, a paid for advertisement under
the definition?
