[cc-community] CC Taiwan release PDF form for CC licenses "agreement"

Hi all,
Yes, we know a CC license does not need an additional "agreement form."
But when we work with government agencies, we found that most agencies
require authors/copyright holders signing something on paper for them to
archive, even the work they using already been licensed under a CC
license.
For these government agencies and people related to them, a agreement
present CC licenses will be helpful. So we made following PDF forms:

CC BY-NC-SA 2.5 Taiwan: http://creativecommons.org.tw/files/agreement/by-nc-sa.pdf
CC BY-NC-ND 2.5 Taiwan: http://creativecommons.org.tw/files/agreement/by-nc-nd.pdf
CC BY-SA 2.5 Taiwan: http://creativecommons.org.tw/files/agreement/by-sa.pdf
CC BY-ND 2.5 Taiwan: http://creativecommons.org.tw/files/agreement/by-nd.pdf
All six forms are written in both Traditional Chinese and English. The
types of work are set according to our copyright law. Since they are PDF
forms so that users can fill forms on computer then print, or just print
then fill with hand.
Optional, people can submit their license information to our archive via
email, helping us gathering some usage statistics anyway.
We don't know how many countries may encounter the same situration/need,
but we still provide our work here to help anyone in such a condition.
Feel free to modify these PDF forms to meet your own need.
