That's not how it works, take a look at REJECT_MSG. If the user says 'n'
to the query the spell won't be cast, whether it has an UNATTEND_SAFE or
not. If the spell is installed, the user obviously had said yes to cast
it the last time. In that case, an update just works if the spell has
UNATTEND_SAFE in it.
There's really nothing magic in there, just add UNATTEND_SAFE to any
rejected spell that works without user intervention (other than the
reject query).
