I filled out this paper for my daughter today. I needed to know the address
on the original registration. Knowing that, I chose the address for the
ballot
to be mailed to , including PO boxes. No Id required for
this process, and they let me fax it in so no way to see I was not my
daughter. If I did not know the address of the original registration, I could
check
a box and the ballot would be sent to the registered address.

In Pennsylvania you are allowed to have an absentee ballot mailed to a
PO Box, but to apply for the ballot, you have to give the actual address
where you live, along with a lot of other information.
