Today I pulled some more & discovered some small brown
worms that I am guessing are the cause of the problem.
Bob suggested onion maggots. You might fight these the same way people deal with carrot rust fly, using row covers.
Another thing it might be is wireworms... about 3/4" long, 1/16" across, light brown, kinda crispy. These are click beetle larvae and I get them eating the stems of my lettuce at ground level, killing the plants. Haven't yet found a good control :-( Luckily they are not anywhere near so bad at my new place as the old... but here the slugs are a hundred times worse to make up for it!
