Erm... this is dead easy to do:
If your USB drive is sda
cfdisk /dev/sda - create one primary bootable partition
mkfs.ext2 /dev/sda1
mount /dev/sda1
cast syslinux
mount the iso image -o loop
copy contents to the mounted /dev/sda1
rename syslinux.* files on /dev/sda1 to extlinux
cat /usr/lib/syslinux/mbr.bin > /dev/sda
run extlinux --install /mounted/sda/boot(or whatever the dir of the
extlinux.* files is)
umount
boot from usb drive
done :)
Note that this can also be used to use extlinux as one's bootloader on a
normal hdd(infact I use this for my sm install on the eeepc)
