Janet,  At our farmers market, (Southern Kentucky Regional Farmers Market)
we use the original farmer application form, where they have listed what
they plan to sell and in what quantities.  At the top of the form is a box
for "office" use, where the person (board member) inspecting the farm checks
off and approves.  You can see the form at our farmers market website:

We then make a copy of their approved application, attach any state
certificates to it, such as a copy of HB391 which allows them to sell
certain baked goods, a copy of the state form which allows them to serve
samples, etc, and put it in a note book for us to have at the market.  Works
well for us and cuts down on the amount of paper.

This is a question for those of you who are members of producer-only 
farmers' markets.  Does your market have a site visit form that is used 
when a manager or someone visits the farm to verify that the produce to 
be sold at the market is grown on the farm?  If so, can you share the form?
