[freetds] DBLIB on Win32

Hi Freddy,
I wanted to have the php extension fully contained with no need for extra
dll's. I think we can add multiple targets to build static or dynamic
libs.
The multithread dll makes compilation with php easier, as that removes
conflicts with libc and libcmt.
