[cc-licenses] any examples of how to attribute?

I have published some sheet music at the www.mutopiaproject.org under
"Creative Commons Attribution 2.5"
( http://creativecommons.org/licenses/by/2.5/ )
An example of those tunes is:
"Traditioner af Swenska Folk-Dansar, 1st part, nr 31"

Last week I was contacted by someone who wanted to create derivative
work ( also sheet music ) of this music. So the license requires him
to attribute my work in his derivative work.
I have read

but I would be very interested in seeing some examples of how such an
attribution text could look like.
Could I require that the attribution text should be placed under each
of the tunes he creates or could I only require that the attribution
text is placed at one place in the book if he publish a collection of
these tunes?
