The wired CD just has the CC logo, a one-sentence plain-english
description of the licenses, and a URL to the license page for the CD.
IANAL, but I'd recommend doing more than that.
So have at least a license notice ("This recording is licensed under
the Creative Commons CC-BY-SA licence.") and the URL of the licenses
along with your (c) and (p) notices on the inlay and on the CD or
record. For publicity you might like to have the (cc) logo on the back
cover (if that's allowed). And for completeness if you have the budget,
reproduce the text of the license inside the inlay.
