[cc-community] CC consent forms

Hi
I'm newish to this list, so I don't know if this has been discussed.
I teach an online film-making course, and require my students to produce
creative commons media for the course. This has the benefit that they need
to get their heads around copyright, the different CC licences and how the
rights put on any third party media they use (eg soundtrack) affects the
licence on which they can publish their own work.
We provide them with a model consent form to use with talent (ie the form
that anyone who appears onscreen or is heard in a film should sign to
indicate they've given their permission to do so), and I've looked in vain
for consent forms that explicitly give permission for performances to be
used for creative commons. I've also struggled to find anything reasonably
easy to understand and many look downright scary (In the UK many consent
forms mention surrendering moral rights, which makes sense to a lawyer
perhaps, but not to your average vox-pop interviewee).
So the question is whether there's scope for developing some of the
paperwork that a media producer uses, which links contributions to CC
licences, and which are human and machine readable in the same way the
licence terms are?
