This is really easy to answer but you need to know the country in which the work is created and the acts are taking place.  The answers to this are not provided by CC licences but by the copyright laws in place in most countries in the world.  Some, such as in Australia, provide quite detailed and codified answers to most of the questions raised.  In other countries, the legislation is much more simple and it will be necessary to rely on general principles to work out the answers.

So, I make a sculpture of a dog from my mind (no preliminary drawings etc.)
and put it in a public park with a BY-SA license. John comes along and takes
a photo of the sculpture and Mary comes along and does a sketch of the
drawing on paper. Mary then scans in her sketch. John posts his picture on
the net under a BY-SA license, Mary does the same with the scan of her
sketch.

What are the source requirements on everyone?
