[cc-community] Using CC-BY-SA in web database editable by multiple users

Hello,
Sorry if this question it stupid, but i don't know where to find such
information.
I am working on creating website with database of free (open source) games.
Content of this website will be created by users, which will be having
possibility
to add new games/releases.
I would like to license all original content (created by users) with
cc-by-sa 3.0.
And i have a question. There is such situation:
1) User xxx adds new game by filling form with such information as
title, description, etc.
Game waits for acceptation by moderator.
2) User moderator1 accepts this game and fixes couple of typos in
description.
3) After one month user moderator2 edits game information because it
isn't actual.
Now, who is author of such work? User xxx only or user xxx, moderator1
and moderator2.
If it's a second case, do i need to provide information about every user
who changed this game information?
Also do i need to provide full information about changes? Something like
diff feature of wiki software?
