I'd suggest having a separate page that just lists the CC license permission.
Have either both parents, or all legal guardians sign that page.
Also have a page that clearly explains why you selected the license
that you did, and how it "protects" the individual.

This is country specific.

Three things to consider:
*  Performer's rights.  (The person who says the speech.);
*  Author's rights.  (The person who writes what is said.);
*  Compilation rights.  (The person who edited the content into a
synthetic whole.);
There are a couple of other rights that come into play, but those are
the crucial ones.

Any third party audio or AV track that is played, falls under a
different set of rights, which also affects the copyright on the
entire presentation.

Consider why you want that license, and if those reasons really apply
to that license.
