Great! (Does anyone disagree?)

But this illustrates where we still fall short I think and points us towards 
needed improvements.

I know I have been bringing up this issue for a good while now and I certainly 
don't recall anyone saying anything other than in-band and "nearby" was 
acceptable.

This has never seemed "sensible" to me and so I have kept asking once in a 
while.

Do we need some sort of "best practices" documents?
