I don't know how it works when you roll them into one like that.
I usually put the notice on a line by itself, because the format
for the notice is very specific. And then I put the license on
line by itself right below that, followed by a list of whatever
information is provided for purposes of Attribution.
Page 1 of Bounty Hunters does just that.

If you provide a URL for attribution, the URL is supposed to
point directly to the license information for that particular
work, not just the front page of your website. In that situation,
I indicate the license first, then list the title of the work
and a copyright notice. For example.
http://www.greglondon.com/cc/by/index.htm
I'm pretty sure these are recognized formats.
There are probably other ways to do it though.
But I am not a lawyer and this is not legal advice so....
