It's meant to apply to all of those cases. There's been some discussion
about whether it actually can do that.

Some edge cases that aren't too clear:

1) You cut a film to match the timing in a song, but you don't combine
the song with the film (DIY music video). Do you have to ShareAlike or
not? (I think you don't, because it's a separate work).

2) What if you combine a song with a film in a video file, but there is
no apparent significance to the timing relationship (e.g. maybe it's
just a slide show of your family photos and you have music playing over
it). (I think you do, because it can be argued that the timing
relationship is fixed, and you specified it, even if there is no obvious
relationship).
