I have been contacted by a few small/medium-sized projects wanting to use
my CC-licensed photos off Flickr. One asked for permission without
reference to the CC license. Another mentioned the CC license, but evinced
concern about CC licenses' non-commercial clause (my license did not use
NC). I informed both that permission was already granted, per the license,
and proceeded to how I would like to be attributed.
On the other hand, others have re-used my photos without asking
permission, attributing me per the license and notifying me after the fact
(out of courtesy). Others have contacted me in advance to ask how I would
like to be attributed.
I think both (a) and (b), as suggested by Alek, hold true for many users,
large as well as small. It is certainly not for lack of clear and complete
documentation at creativecommons.org.
